FBA Documentation Template + Full BIP
FBA_Documentation_Template-SSIT2026-MTSS_Model.docx  /  Full_BIP-SSIT2026-MTSS_Model.docx
Required for Tier 3 behavior scholars · FBA must be completed before BIP can be written · Build only when there is an active Tier 3 behavior scholar — EC staff should be involved in the design since they are the primary users
Intensive Meeting Template (blank, reusable)
Intensive_Meeting_Template-SSIT2026-MTSS_Model.docx
For monthly Tier 3 SSIT reviews · A completed example from March 2024 exists in the Drive folder · Clean it up, blank it out, and save as a reusable template when a Tier 3 scholar enters the monthly review cycle
T2R2 Meeting Template (clean blank version)
T2R2_Meeting_Template-SSIT2026-MTSS_Model.docx
The March 2025 Supplemental Support meeting record is the best version of this template · Remove scholar-specific content, replace PowerSchool with Infinite Campus, add “provide teacher with SSIT Folder Checklists” at Step 5 · Build when the current informal template is causing confusion
When do I need SSIT — and when do I manage it myself?
Teacher manages independently
✔ Tier 1 instructional adjustments within DI groups
✔ Tier 2 Round 1 “Spot” — triggered by 3+ CBA failures
✔ CICO / Daily Pride Report at Round 1 (behavior)
✔ Color chart escalations (Yellow → Orange → Red)
✔ Informal parent contact logged in Infinite Campus
Documented in PLC notes and teacher data binder only
SSIT involvement required
✔ Any Tier 2 Round 2 (“On-going”) placement
✔ Any Tier 3 placement
✔ Any Mini-BIP or Full BIP development
✔ Any Section 504 or EC referral consideration
✔ Any parent-initiated request for formal review
Complete Academic or Behavioral Investigatory Form first

NC Parent Notification: Round 1 requires informal contact logged in Infinite Campus only. Round 2 and above require the formal Parent Notification Letter (in Parent_Notification_Letters-SSIT2026-MTSS_Model.docx) before intervention begins. This distinction is intentional and aligned to NC DPI guidance.
