Public School Health Assessment
NC State Law requires that every child entering public schools in N.C. for the first time receive a health assessment. The medical provider, parent or guardian must provide a complete health assessment transmittal form to the principal of the school on or before the child’s first day of attendance.
Your Doctor’s Office / Pediatrician is aware of the need for a NC Health Assessment Form and have them at their location. When you call to make your doctors appointment let them know that your child will be attending school in Fall and needs a copy of their Health Assessment.

K-12 School requirements
North Carolina Law requires all children in the state to receive certain immunizations. Records are checked when children enter school or child care.
